AATI SPECIAL WRITTEN CERTIFICATION EXAM SESSION REGISTRATION, POLICIES AND PROCEDURES

1. Registration Process: To register for the AATI SPECIAL Written Certification Exam Session, please complete the provided registration form. You may register between 8:30 AM and 8:30 PM, Monday through Thursday (closed on Fridays). Payment can be made by major credit card, debit card, Company Checks or money order.

2. Exam Date and Location: Your exam date and time are indicated on the other side of this page. Please arrive at 6801 W. 20th Ave, Hialeah, 33014 at least 15 minutes before your scheduled time. If you are taking a recertification exam, we will verify your eligibility. Be sure to provide your full name exactly as it appears on your current (unexpired) government-issued photo ID, which you will need to present on the exam day.

3. Terms of Registration: By registering for the AATI Written Certification Exam, you accept the terms regarding the exams and reporting of results, including AATI's right to confirm your current certification status and areas of certification. You also authorize AATI to release your information to relevant counties, which may include, but is not limited to, exam certification scores, application status, expiration dates, continuing education status, and information required by county regulations.

4. Exam Availability: You may take up to two (2) certifications exams or you may take up to three (3) recertification's exams during this session, subject to scheduling and space availability. Registration must be received no later than the stated "Register By" due date. Space is limited and provided on a first-come, first-served basis.

5. Testing Schedule: You will be scheduled for one, two or three exams within a four-hour period. It is essential to arrive on time. Once the exam session begins, no late entries will be permitted.

6. Confirmation: Please have a valid email address available at the time of registration.

7. Responsibility for Exam Date: You are responsible for knowing your exam date. Missed tests cannot be rescheduled. If you misplaced your copy registration, please call us at 305-362-5519 or email us at admission@aati.edu. Remember, it is YOUR RESPONSIBILITY.

8. Missed or Failed Exams: If you miss or fail the exam, you must register again and pay the applicable fee. Late arrivals will not be allowed to take the exam and will be required to pay again to schedule a new appointment. You may take an electronic exam if you miss or fail the written exam.

9. Fees and Refunds: Fees are non-refundable, and no credit is provided for missed exams, including late arrivals.

10. Identification Requirements: You must present a current (unexpired) government-issued photo ID. The name on your registration must match the name on your ID. If you recently changed your name (due to marriage, divorce, etc.), you must provide an official legal document (e.g., marriage license or court order) to verify the change. Acceptable forms of ID include a driver's license, state-issued ID, work permit, military ID, government-issued ID, resident alien ID, passport, police ID, firearm owner registration ID, and school-issued ID. All ID cards must be current and include a photo. If you are unable to test because you do not present an acceptable photo ID, you will forfeit all fees.

11. Prohibited Items: Electronic devices such as cell phones, calculators, tape recorders, smartwatches, smart glasses, cameras, or any other devices capable of recording, sending, or receiving information are not allowed in the testing room. You may not bring any aids, manuals, books, scratch paper, pencils, or notes into the room. Personal items such as books, hats, and bags are also not allowed in the testing area.

12. Guests: Guests are not permitted.

13. Food and Drinks: Food and drinks are not allowed in the testing room.

14. Other Prohibited Items: Weapons, valuables, backpacks, briefcases, large purses, and other miscellaneous items are not allowed in the testing room. AATI is not responsible for lost items.

15. Score Reporting: You will receive an official, detailed score report within 30 days of your exam date. If you pass, a certificate will be issued.

16. AATI Rights: AATI reserves the right to cancel any test score or revoke certification if there is any reason to question the validity of a test score, suspect fraudulent registration information, or suspect misconduct during the exam session.

17. Certification Statement: By signing this application, you certify that you are the person named on page 1 of this form and accept these policies and procedures. You understand that examination material is confidential and may not be reproduced, shared, downloaded, printed, copied, or used in any form.
